ENHANCEMENTS:
1) Support for ESX 4.1

3) Switches to Unix/Windows receive buffer interface for
   improved performance.
4) Added page-based receive & driver private LRO as an experimental
   feature.  Load driver with myri10ge_rx_skbs=0 to activate.
5) Added a new option (myri10ge_allow_degraded_pcie) to allow driver
   to still operate with a degraded PCIe link.
6) Allow myri10ge to selectively attach to NICs based on MAC address.
7) Support for ethtool --identify
8) Added new option (myri10ge_esx_autotune) which adjusts the number
   and size of receive queues based on the number of NICs installed.
   When enabled (default), this option will override the module
   parameters myri10ge_skb_limit, myri10ge_max_slices and 
   myri10ge_small_bytes.

BUG FIXES:
1) Fixed 2 bugs which could lead to PSOD or hang under heavy 
   receive load.
2) Fixed a bug which could lead to spurious watchdog resets.

1) Use 4 NetQueues by default to prevent rx packet buffer
   shortages when multiple NICs are in use.
2) Limit each ring to 128 RX buffers to work around
   rx packet buffer shortages when multiple NICs are in use.
3) Fixed issues with changing the MTU under load.
4) Fixed crash triggered by netdev tx timeout.
